Unified Facility Program Permit Application:
The application package and instructions help a business determine whether they are in compliance with six programs that control and monitor the use of hazardous substances. To streamline applicant efforts, the six programs have been consolidated into one application package. To streamline the efforts of regulating agencies, the application is processed by one agency, the local Certified Unified Program Agency (CUPA). In 1997, the Los Angeles Fire Department was certified as the City's Unified Program Agency. In addition, the application package has been standardized, so that many of the local Fire Departments use the same package.
When the application package is completed and your hazardous waste/material activity is declared, you will know whether you need a Unified Program Facility Permit, a U.S. EPA hazardous waste generator number, a completed inventory of hazardous substances, a Business Plan, a California Accidental Release Prevention Program Plan, underground tank permits, as well as onsite reclamation, treatment, disposal or recycling activity reports or permits.
If outside the City limits, contact the appropriate Certified Unified Program Agency (CUPA) for a similar application package. CUPAs outside the City of Los Angeles are usually the local Fire or Environmental Health Depts. For example, listed below is the local CUPA for most businesses located outside the City of Los Angeles, but within Los Angeles County:

Hazardous Substance Inventory Inspections:
Hazardous substances (materials and waste) inventory inspections are typically performed by the data management unit, as listed above. At the discretion of the Chief, inspections may be conducted for the purpose of determining compliance with this division and to cause to be corrected any information which is discovered to be either different from information in the application for a Permit or HMRRP authorization or any amendment to the Hazardous Materials and Hazardous Waste Inventory Form filed with the City, or inaccurate, concerning the handling of hazardous substances at a business site.This inspection must be performed once every three years per State law. The unit inspectors will verify that the hazardous substance inventory is correct and that the appropriate "Unified Facility Program Permit" paperwork is in order. Depending upon the type of business, this may include information on safety training, evacuation plans, emergency contacts, site maps, etc.

Underground Tank Inspections:
Underground hazardous substance storage tanks are inspected by the Underground Tank Unit, with assistance from the Engineering Unit, as necessary. This includes Division 5 permitting, installation, operation, removal, contamination assessment and clean-up, as necessary.

Hazardous Substance Fire Safety Inspections:
With the exception of underground storage tanks and hazardous substance inventory inspections, as described above, the City Fire Depts. industrial/commercial unit may also perform inspections to ensure hazardous substance storage and practices are in conformance with applicable fire codes and best management practices.

Hazardous Waste Management Inspections:
The Los Angeles County Fire Department, HHMD is a partnering agency with the City CUPA (LAFD Bureau of Fire Prevention and Public Safety). The LAFD has entered into an agreement with the L.A. County Fire Department to perform Hazardous Waste components of the Unified Program. They verify that businesses are managing their hazardous waste in conformance with State and Federal hazardous waste management regulations.

Specific occupancy requirements apply to businesses that generate, store, or treat hazardous substances. Construction of associated buildings, structures and areas also have specific guidelines that must be followed. The Department of Building & Safety and the Fire Department Construction Services Unit enforce these requirements, as defined in the City of Los Angeles Building Code, the Uniform Fire Code, and local ordinances.
